Who qualifies for Hospice Care?

There are two criteria to qualify:

  • The Patient must be referred by a physician who certifies prognosis as 6 months or less.

  • The Patient must elect Hospice Care.

Who pays for Hospice Care?

For qualifying individuals, Hospice Care may be covered under Medicaid or Medicare. In other cases, it may be covered by private insurance.

What is Hospice Care?

Hospice is end-of-life care for those diagnosed with a terminal illness. It is highly personalized care focused on keeping the patient comfortable, providing medical attention that addresses symptoms, and providing necessary daily-living help.

Hospice care is about the quality of each day, not the number of days. Many people seek hospice care when their illness has a terminal prognosis. Hospice care can also provide services to individuals who may need additional emotional support, or who decide to discontinue treatment and are preparing to spend their final days at home.

What services are included with Hospice Care?

What makes Hospice unique is the team approach to care. An individual program is developed for each patient, in conjunction with the referring physician who specifies the medical care to be delivered.
Our social worker spends time with the patient and their family to identify the care that the patient wants, and communicates their wishes to the rest of the team.
